Incident Management

Every report, from triage to root cause

Workers report through My ReFresh, a QR code or a shared link, anonymously if they choose. Each incident then moves through triage, investigation and response, with the next step spelled out.

Reports come in however workers are comfortable

Workers report in My ReFresh, through a QR code or a shared reporting link, or HR logs it for them. Anonymous reports stay anonymous, and an admin can request identification if it is needed.

A next step on every incident

Each incident follows one path: reported, triage, investigation, response, resolved, closed. A banner on the record says what has to happen next.

Investigations that find the root cause

Run the investigation inside the incident. Collect statements from everyone involved, then record the findings, the root cause and the contributing factors. The investigator can be internal or an outside firm.

Classified by hazard, rated by severity

Tag each incident with its event type, primary hazard and any other hazards, then rate severity, potential severity and immediate risk. Patterns show up by hazard and by team.

An incident linked to its investigation, risk, control and consultation

From one report to a better control

Link the incident to the risk assessment, controls and consultation that follow it, so the response sits on the same record as the report.

GOT QUESTIONS?

Frequently asked questions

How do workers report an incident?

Through My ReFresh, a QR code or a shared reporting link. HR can also log an incident on a worker's behalf. Every report lands in the same incident register.

Can workers report anonymously?

Yes. An anonymous report shows as Anonymous on the record. If more detail is needed to act on it, an admin can request identification from the record.

What happens after an incident is reported?

It moves through six steps: reported, triage, investigation, response, resolved and closed. New reports wait in the triage queue, and a banner on each record tells the admin what needs to happen next.

Can an outside investigator work in ReFresh?

Yes. An investigation can be run by an internal investigator or an external firm. Statements, findings, the root cause and contributing factors are all recorded inside it.

How do incidents link to our risk register?

Each incident has tabs for its investigations, consultations, controls, risk assessments and tasks, so the response stays linked to the risk it relates to.

How fast can we get set up?

Most organisations move through setup quickly:

  • Week 1: configure your organisational structure, activate compliance frameworks, and import existing documentation.

  • Weeks 2–4: full compliance mapping including risk scenarios, first survey, and control setup.

  • Ongoing: continuous monitoring, automated legislation updates, and board reporting from day one.

We also offer a hands-on onboarding service where the ReFresh team comes in (online or in person) and sets everything up with you, including HRIS integrations.
